Two dozen photos in the DEP inspection report document the violations along the entire route located in Tuscarora Township, Bradford County.
Among the violations were--
-- Multiple holes and tears in the pipeline liner;
-- Pipeline was installed on top of erosion & sedimentation controls making them ineffective;
-- Failure to install a shut off valve;
-- Failure to install the pipeline on the route shown in the permit;
-- Failure to stake out and protect wetlands along the route;
-- Failure to use timber mats to cross wetlands, tire tracks were seen going through the wetlands;
-- Failure to install temporary erosion & sedimentation controls during construction; and
-- Failure to hold a required preconstruction meeting.
