Enforcement Actions Taken Last Week

Mine Subsidence Found Under Compressor Station Construction Site


Hyperion Midstream LLC - Incident: Filling Mine Subsidence Void At Compressor Station Construction Site

On March 7, Olympus Energy/Hyperion Midstream LLC notified DEP it found an abandoned coal mine subsidence void under the construction site for its Guardian Natural Gas Compressor Station off Wilderness Road in Trafford, Penn Township, Westmoreland County, according to DEP Oil and Gas Program inspection reports.

Olympus/Hyperion said they had been aware of the “potential to encounter subsidence at Guardian because of the extensive geotechnical work that has been done on the site,” according to DEP’s report.

The DEP permit for the compressor site required Olympus/Hyperion to notify the agency “immediately by phone” if geologic formations or soil conditions are encountered that might cause pollution or if active or abandoned deep mines are encountered.

Olympus/Hyperion submitted a geotechnical report to DEP’s Oil and Gas Program and the Bureau of Abandoned Mine Reclamation on the mine void with a proposal for how to fill it in and prevent future mine subsidence.

A diagram submitted to DEP shows the mine void is narrow at the top but extends down to an old room-and-pillar mine whose roof has partially collapsed creating a void.

The report recommends carefully monitoring the entire construction site for additional mine subsidence activity and to institute subsidence mitigation measures as needed.

On March 21, DEP notified Olympus/Hyperion its plan to fill in the void was approved.


Explosion & Fire At Conventional Gas Well


Kriebel Natural Gas Co LLC - Violations: Wastewater Release

On March 19, 2025 DEP inspected the RWS Development 001 conventional well in Manor Borough, Westmoreland County in response to a notification of an explosion and fire at the well site March 19 with the Manor Volunteer Fire Department, Westmoreland City Fire Department and the well owner.

The conventional well, owned by Kriebel Natural Gas Co., LLC, exploded and caught fire shortly before 1:00 p.m. on March 19, according to the Manor Volunteer Fire Department Facebook page.

DEP arrived on the scene just before 2:30 p.m. and met with the Manor Volunteer Fire Department, Westmoreland City Fire Department and representatives of the well owner to help assess the site to determine any environmental impacts..

At the time of DEP’s arrival, the well owner was attempting to shut down a gas leak from the well casing annulus and remove the remaining wastewater from a damaged storage tank at the well.

DEP said in its inspection reportIt appears that a complete [wheel] rim set with a brake drum and tires had come off a trailer being hauled down [the road] which then came in contact with the well. 

“That impact broke off the [well] casing annulus wing from the wellhead and caused the escaping gas to then ignite.”

The first responders reported using 3,200 gallons of water mixed with Dawn dish soap to extinguish the fire.  

Damage to the well equipment included a melted hole in the wastewater storage tank that leaked fluid around the tank and down a drainage way for about 180 feet, but there did not appear to be an impact to a stream, DEP said.

March 10-13, 2025 inspections of 26 conventional wells in Baden and Economy Boroughs, Beaver County found them all abandoned and not plugged. Other violations include leaking gas and failure to submit annual production, waste generation and well integrity reports.  Most of the original violations were issued in November 2023. March 18, 2025 inspection of the Metcalf 1 [DEP inspection report] and Metcalf 2  [DEP inspection report] conventional wells in Plum Borough, Allegheny County found the wells abandoned and not plugged and were leaking gas.  Owner also failed to submit annual production, waste generation and well integrity reports. The landowner put fences around both wells for the safety of others.  Violations issued.  Response requested by April 1.  Leechburg Gas holds 16 permits.


Charles E. Myers - Violations: Abandoned Conventional Well

March 18, 2025 inspection of the WM T. Baum 1 conventional well in Clinton Township, Venango County found the well abandoned and not plugged.  Owner also failed to submit annual production, waste generation and well integrity reports.  Owner submitted a notice of intention to plug the well which is scheduled to start on March 19.  Violations issued. No response requested.  DEP inspection report.   Charles Myers holds 4 permits.


3D Resources LLC - Violations: Abandoned Conventional Well

March 12, 2025 inspection of the WIANT 001 conventional well in Redbank Township, Clarion County found the well abandoned and not plugged. The owner also failed to submit annual production, waste generation and well integrity reports. Violations issued. Response requested by April 10.  DEP inspection report3D Resources holds 7 permits.


PBS Coals Inc. - Violations: Abandoned Conventional Well

March 14, 2025 inspection of the Victor Sinemus 1 conventional well in Green Township, Indiana County found the well abandoned and not plugged. Owner also failed to submit annual production, waste generation and well integrity reports.  DEP notes PBS filed for bankruptcy on January 6, 2025.  Violations issued. No response requested. DEP inspection report.


Edward E. & Frederick J. Craig: - Violations: Abandoned Conventional Well, No Reports

March 17 & 18, 2025 inspection of the Helen Jameson 4 conventional well in Fairview Township, Butler County found the well abandoned and not plugged.  Owner also failed to submit annual production, waste general and well integrity reports.  DEP inspection report.

Inspections of other wells by the same owner in the same area on the same days found failure to submit  annual production, waste general and well integrity reports and other violations: John Jameson 1, John Jameson 3, JE Knoll 7, JE Knoll 8, Ruben Holt III 5.

7 Shale Gas Abandoned Wells


Blackhill Energy LLC -  Violations: 4 Abandoned Shale Gas Wells

March 4, 2025 inspections of four shale gas wells in Bradford County found them all abandoned and not plugged. Violations issued.  Response requested by April 11 to 18. 

-- Springfield Twp.: Lee 1H was also leaking gas. Inactive status expired on February 7, 2019. Drilled in 2011 but not produced.  [DEP inspection report]  

-- Springfield Twp.: Kline 11H. Inactive status expired on April 22, 2019. Drilled in 2013 but not produced.  [DEP inspection report]

-- Ridgebury Twp.: Fay 1H. Inactive status expired on February 21, 2020. Drilled in 2011 but not produced. [DEP inspection report]

-- Ridgebury Twp.: Reiter 1H. Inactive status expired on February 21, 2020. Drilled in 2010 but not produced. [DEP inspection report]

Blackhill Energy holds 90 permits.


EQT ARO LLC - Violations: 3 Abandoned Shale Gas Wells

March 4-6, 2025 inspections of three shale gas wells in Centre and Lycoming counties found them abandoned and not plugged. Response requested by April 11.

-- Centre County, Snow Shoe Twp.: DCNR COP Tract 231 A-1000H. Inactive status expired November 7, 2020. Drilled in 2009 but not produced. [DEP inspection report]

-- Lycoming County, Campbell Twp.: Clearview Hunting Club Pad A 4H. Inactive status expired July 1, 2021. Drilled in 2011 but not produced. [DEP inspection report]

-- Lycoming County, Cummings Twp.: Larrys Creek F&G A 6H. Inactive status expired on July 1, 2022. Drilled in 2011 but not produced. [DEP inspection report]


Shale Gas Well Plugging Problems


EQT Chap LLC - Incident: Problems Plugging Inactive Shale Gas Well

March 5, 2025 inspection of the 7H shale gas well at the Basista-Dermotta well pad in Sewickley Township, Westmoreland County in response to a notification by owner found they were running into significant problems plugging the well [DEP inspection report].   

The crew was washing the hole and equipment became stuck at 4,200 feet before they were able to reach 7,400 feet.  Problems with stuck equipment and clearing the well to complete plugging are continuing [March 21 inspection report].

DEP granted inactive well status to this well on November 7, 2019 when the well was owned by Chevron Appalachia, LLC.

“The operator has drilled 3 wells on the pad to approximately 2,400' and plans to complete the three wells in the future. The operator plans to restore the site to the interim restoration plan in 2020 with drilling activity to resume at the site in 2024, with completion of the well allowing for production of the well in the future.”

“The pressure gauge on the well head is reading over 8,000 psi and appears to inoperable.”  The owner said it would be replaced.

On February 20, 2025, DEP approved an alternative method of plugging the well-- “In lieu of venting requirements related to wells located in coal areas, EQT will cement the well from total depth of attainable bottom to the surface.”

The well was originally drilled on July 6, 2012, but never completed.

In 2024, 47 violations were issued or continued to 12 shale gas well owners for abandoning and not plugging their wells.

Raymond D. Langer - Violations: Conventional Wastewater Release (14 Months Old)

March 19, 2025 inspection of the Lorish 1 conventional well site in Robinson Township, Allegheny County found continuing evidence of the impacts of a wastewater spill, erosion and sedimentation issues, down trees and branches and unused production equipment. No action was taken to clean up the spill.  Original violations for wastewater release were issued January 11, 2023.  Violations continued, new ones coming.  Response requested by March 27.  DEP inspection report.


Raymond D. Langer - Violations: Conventional Black Fluid Release (14 Months Old)

March 19, 2025 inspection of the Choeller 1 conventional well site in Robinson Township, Allegheny County found continued evidence of a black, petroleum-based substance release around the wellhead and surrounding area and a leak from one of the production tanks.  Black fluid was also pooled in the earthen berm around the tanks. No action was taken to clean up the spill.  Original violations issued for these failures on January 11, 2023.  No response requested. DEP inspection report.


Raymond D. Langer - Violations: Conventional Black Fluid Release (14 Months Old)

March 19, 2025 inspection of the J. Riddle 2 conventional well site in Robinson Township, Allegheny County found continued evidence of a black, petroleum-based substance released around the wellhead and surrounding area. Black liquid was also in the area surrounding the production tank, through the containment berm and downslope from the oil derrick. No action was taken to clean up the spill.  Original violations issued for these failures on January 11, 2023.  No response requested.  DEP inspection report.


Danken-Sammar LLC - Violations: Wastewater Release (Nearly 3 Months Old)

March 7, 2025 inspection of the Green Glen 1 conventional well in Huston Township, Clearfield County found continuing evidence of a wastewater release around the well, although some work was underway to excavate the contaminated area. Samples were taken. Original violations issued on December 13, 2025.  No response requested. DEP inspection report.


Fawnwood Energy Inc. - Violations: Conventional Wastewater Release

March 19, 2025 inspection of the Richard R. Walkers Et Al 27 conventional well in Bruin Borough, Butler County with the well owner in response to a complaint. The well owner said he found the well bore had flooded and discharged approximately 420 gallons of fluid to the nearby pond/wetland between March 15 and 18.  He also said  he placed booms and absorbent pads around the perimeter and across the surface of the pond as soon as he discovered the release.

“There is no obvious outlet to the pond/wetland and the discharge appears to be contained and controlled. The well owner plans to continue placing and replacing mats and booms to absorb as much fluid as possible. He said that he will continue to monitor the situation and work to develop a continued action plan.”

A response was requested by April 5.  DEP inspection report.


Deitz Gas & Oil Inc. - Violations: Conventional Oil Running Down Side Of Tank

March 13, 2025 inspection of the Mahle M12 conventional oil well in Highland Township, Clarion County found oil flowing down the outside of a production tank and contaminating the soil around the tank. The well is on a wooded, unreclaimed coal mine. Violations issued. Response requested by April 10.  DEP inspection report.


Consortium Energy LLC - Violations: Release Of Conventional Crude Oil

March 6, 2025 routine inspection of Warrant 3410 P19 conventional oil well in Lafayette Township, McKean County found a failure of an oil gathering pipeline resulting in the release of an unknown quantity of crude oil and wastewater into a ditch where it traveled 210 feet until entering a catch basin installed by the owner to catch the release.  There were also several pools of oil. When weather conditions allow, more work needs to be done to characterize the extent of the release and cleanup measures.  Violations issued. Response requested by March 25.  DEP inspection report.


Range Resources Appalachia LLC - Violations: Failed to Put Oil, Antifreeze In Containment

March 19, 2025 inspection at the Ziolkowski 11771 shale gas well pad in Findlay Township, Allegheny County found totes containing antifreeze and hydraulic oil were stored out of secondary containment to contain any spills.  Totes were moved. Violations issued, closed.   DEP inspection report.


Petro SVC Partners Inc. - No Violations: Well Leaking Gas

March 20, 2025 inspection of the Earl Davis 1 conventional gas well in Hempfield Township, Westmoreland County found the well leaking gas.  Recommended the owner fix the leak.  No violations issued. DEP inspection report.


Land Recycling/Brownfield Cleanups


So far in 2025, DEP received or acted on 29 Act 2 Land Recycling notices related to oil and gas facility site cleanups. Read more here.


Failure To Submit Shale Gas Production/Waste Reports


On March 19, the Department of Environmental Protection issued notices of violation to Range Resources Appalachia LLC and LPR Energy, LLC for failing to submit required monthly reports of natural gas production, waste generation and disposal for shale gas wells in 12 counties.

Five other companies were also cited for the same violation: Ows Energy, LLC, PPG Oper LLC, Baker Gas Inc., John E. McCool and Media Res Dev Co. LLC.

The Range Resources notice of violation covered shale gas wells in Allegheny, Beaver, Butler, Clinton, Lycoming, and Washington counties for the January 2025 reports.

The LPR Energy notice of violation was for shale gas wells in Bedford, Blair, Centre, Clearfield, Fayette, and Somerset counties for the January 2025 reports.

Well owners have 10 days to respond to the violation.

On March 19, 2025 DEP issued a notice of violation for failure to submit the January production and waste generation report for one or more shale gas wells in Oakland Township, Venango County, and East Fairfield Township, Crawford County.  The owner has 10 days to respond.


PPG Oper LLC - Violations: Failure to Submit Shale Gas Production/Waste Report

On March 19, 2025 DEP issued a notice of violation for failure to submit the January production and waste generation report for one or more shale gas wells in Goshen and Girard Townships, Clearfield County.  The owner has 10 days to respond.


Baker Gas Inc. - Violations: Failure to Submit Shale Gas Production/Waste Report

On March 19, 2025 DEP issued a notice of violation for failure to submit the January production and waste generation report for one or more shale gas wells in North Buffalo Township, Armstrong County. The owner has 10 days to respond.


John E. McCool- Violations: Failure to Submit Shale Gas Production/Waste Report

On March 19, 2025 DEP issued a notice of violation for failure to submit the January production and waste generation report for one or more shale gas wells in Brokenstraw Township, Warren County. The owner has 10 days to respond.
